How would you categorise your general physical condition?

I am very fit, energetic and exercise regularly

I am well. I exercise occasionally and have no active diseases

I am managing well. I have some medical problems but they are under control. I don’t exercise apart from walking

I am vulnerable. I do not rely on help from others but I find my symptoms limit what I can do. I feel tired and slow during the day

I am mildly frail. I have slowed down a lot and am starting to find it difficult to do things like shopping, walking outside alone, or housework. I may need help with some things like transportation or sorting out my medication

I am moderately frail. I need help when i go outside and with the housework. Stairs are difficult and i might need help with bathing and dressing

I am severely frail. I need someone to care for me completely
